Preparing for Elder Parents Applying for Medi-CAL Long Term Care

As a Medi-CAL and Elder Law Law Firm, our Medi-CAL Attorneys and staff see adult children every week come into our office and want to get help with Medi-CAL qualifications. And many times their parents do not have the documents required to even apply for Medi-CAL.  Most importantly is their proof of citizenship. This means either their birth certificate showing they were born in the USA , a VALID Green Card or Certificate of Naturalization.  Also they need A VALID State ID… it can be drivers license or state ID. I am amazed at how many people do not keep their ID valid after they can not drive anymore.  This is critical!!!

This is a check list of documents that they may need  (depends on assets):

  • Copies of MOST CURRENT Bank Statements and last four (4) months of statements:
  • For all OPEN accounts, including checking, savings, certificate, etc.
  • Copies of MOST CURRENT Stock Broker Statements and last four (4) months of statements:
  • For all OPEN accounts
  • Last check stub or annual statement for Applicant and Spouse from: Social Security, Veterans Administration, Civil Service, Pension
  • Life Insurance Policies for Applicant and Spouse and statements showing ‘cash surrender values’ for each policy (term policies are exempt, but still need Statement)
  • Long Term Care Policy for Applicant and Spouse
  • Supplemental Health Insurance Policy and Card and recent Bill for Premium
  • Annuity Policies for Applicant and Spouse and statement showing current value and distribution amount, if annuitized
  • Stock Certificates, Bonds, CDs, US Government Treasury Bonds, Municipal, Individual Retirement Accounts (IRAs), or any other deferred comp plans, for Applicant and Spouse showing gross and net payments
  • Copy of any special burial bank account for Applicant and Spouse
  • Copy of any prepaid burial or cremation contract for Applicant and Spouse
  • Copy of Cemetery Plot Deed for Applicant and Spouse
  • Copy of Deed to primary residence and last real estate Tax Bill and Homeowner’s Policy Premium.
  • Copy of Deed to any other real property in Applicant’s name, in Spouses name, or joint names of Applicant or Spouse with any other person along with tax bill
  • Copy of any title(s) issued by Department of Motor Vehicles: i.e. auto, mobile home, boat, trailer, truck, van, recreational vehicle, or other registered to Applicant or Spouse
  • Birth Certificate or other proof of citizenship (naturalization papers, or Passport) for Applicant and Spouse
  • Copy of Marriage License, if currently married
  • Social Security card(s), Medicare Card(s), and supplemental insurance card(s) for Applicant and Spouse
  • Valid Driver’s License of applicant and spouse
  • Current Nursing Home Record of Admissions Sheet( showing Date of Admission) Address and Phone number of Skilled Nursing Facility
  • Current Will, Trust, Power of Attorney, or Other Estate Planning Document for Applicant and Spouse
